Hopewell Valley CHS/Timberlane MS Transportation Release Form | Hopewell Valley Regional School District

**THIS FORM MUST BE FILLED OUT ON A DAILY BASIS, NOT AS A BLANKET FORM FOR THE SCHOOL YEAR**

Please fill out the travel release form below. By typing my name below, I am providing my electronic signature. I certify that I am transporting my child from the event/contest. I understand that it is the policy of the Hopewell Valley Central High School/ Timberlane Middle School Athletic Department that athletes and activity participants ride to and from away events with their team in school-provided transportation and that a departure of this requirement will release the Hopewell Valley Regional School District from any and all liability which may occur as a result of my child not traveling back to Hopewell Valley Central High School/ Timberlane Middle School with the team. Policy also states that parents may only take their own child(ren) home from an event/contest!

Thus, I hereby release the Hopewell Valley School District and its’ employees and officers from any and all liability with reference to the above-stated transportation.
Under all normal circumstances, this form must be on file in the Athletic Office prior to 11:00am the day of the event from which the student is not traveling back to Hopewell Valley Central High School/Timberlane Middle School in school provided transportation.
