IMPORTANT:  You are only registered once ALL steps are completed.
 

All WINTER HIGH SCHOOL SPORTS: 

     Physical Forms and Athletic Registration are due on or before WEDNESDAY, NOVEMBER 6th!  
      

 

In-House Physicals will be offered on Friday, October 11th  for all high school athletes from 9:00 am-12:00 pm in the high school nurse's office.  They are open to students in grades 9-12 by appointment only and are $25 a child.  (Checks should be made out to HVRSD)

In-House Physicals will be offered on Thursday, October 17th for all middle school athletes from 9:00 am-12:00 pm in the TMS school nurse's office.  They are open to students in grades 6-9 by appointment only and are $25 a child.  (Checks should be made out to HVRSD)

Step 1: Athletes must have a current sports physical exam on file in the School Nurses Office.  Please note, physicals are good for 1 year (365 Days) from the exact date of the last physical.  NJ Law requires that a paper copy be kept on file in the school's nurse's office. Please click here to view or download instructions on how to fill out the Physical Form.

IMPORTANT:  Please read carefully

Hopewell Valley Regional School District requires the submission of a comprehensive medical history, evaluation, and clearance. In order for a student to participate in athletics at the middle school or high school, parents/ guardians must submit EITHER the entire state form (including the clearance letter, physical evaluation, and medical history), or the HV Athletic Physical Form.

  • If you choose to submit a copy of the New State Form, you do not have to submit anything else.  Please submit ALL Pages!  
  • If you choose to submit the HV Athletic Physical Form here are the steps that you need to complete.
    • 1.  Your primary doctor will keep the New State Form on file in their office.
    • 2.  Submit the first page (Clearance Letter of the New State Form) along with the HV Athletic Physical Form. 

      Either the New State Form or the HV Athletic Form will be reviewed by our school doctor before the student is permitted to attend tryouts and/or practice.

 

Note: If there is a current physical on file with the school nurse, you are only required to complete the Health History Update Questionnaire Form Please ensure all required signatures are provided.

Once the Preparticipation Physical Evaluation Forms or the Health History Update Questionnaire Form is completed, please submit to the school nurses office.  For your convenience and so that there is a record of the physical on this database, you can also scan it and submit completed Physical Forms online through your family account.     
 

 

 

 

Step 2:  Central High School Registration Form-  This must be completed each season in order to ensure registration for each sport.   

 

Step 2: Timberlane Middle School Registration Form-  This must be completed each season in order to ensure registration for each sport.  

 

 Once you have completed these steps and handed in your medical forms, your coaches will be informed if you have been cleared to participate in your sport. They will communicate this information to you and let you know if you are missing anything.
